About the Role
Provide intensive in-home counseling in one-to-one, family, and group settings.
Emphasize parenting skills training, behavioral, social, and recreational needs.
Implement positive behavior support plans for lasting impact.
Develop personalized support plans tailored to individual needs.
Model and teach effective problem-solving techniques.
Initiate referrals for community-based services, ensuring comprehensive support.
Coordinate with school personnel to monitor and enhance academic achievement.
Schedule appointments with youth and families on a weekly basis for ongoing support.
Offer 24-hour crisis intervention and support as necessary.
Monitor psychotropic medications, providing psychosocial education for medication and recovery.
Document all service delivery in a timely manner as per regulatory standards.
Maintain regular contact with referring agencies and other service providers.
Actively coordinate discharge and aftercare plans with referring agencies.
Attend and participate in interagency meetings, team meetings, and training sessions.
Utilize your valid driver's license to ensure effective community-based services.
Requirements
- Bachelor’s Degree in Social Work, Psychology, or a related human service field.
- At least one year of professional experience in intensive in-home programs or residential facilities.
- Registered with the Board of Counseling as a QMHP-C.
- Excellent written and verbal communication skills.
- CPR and First Aid certified.
- Behavior management certification.
- Must pass a criminal background investigation and registry check by the Department of Social Services.
- Valid driver’s license and current DMV driving record.
